This privacy statement informs you how we handle your data.
@teunschutte is responsible for the processing of personal data.
What data is recorded?
The basic principle for recording personal data is that no more personal data is collected from you than is strictly necessary. They may only be processed for a specific purpose.
The purposes for which @teunschutte collect your personal data are:
Retrieval and contribution of patient perspective and patient experiences and of the experiences in the home situation.
To be able to reach you (call or e-mail) if this is necessary in the context of the study. Think of making an appointment.
Depending on the survey you participate in, we may process the following personal data about you:
Initials, first and last name
Gender
Address
Residence
Telephone number
Email address
Age (exact date of birth is not necessary)
Education, training
Commuting situation
Consent that you want to participate in the study

Special categories of personal data
Special categories of personal data are data about health and data about your social circumstances.
This data helps us to interpret the patient's perspective and patient experiences.
Of course, we only process this special personal data if you have given explicit permission (also known as "consent") for the processing of this data.
We collect this data when you participate in an interview or fill in a questionnaire.
Children's Data
If it is relevant to a project that insights and experiences are collected from children aged 18 or younger, this will be done through a parent or guardian. Candidates will only be contacted through the parent or guardian. The conversation is recorded in a consent statement with the parent or guardian or without, but with the explicit consent of the parent or guardian.

How long do we keep personal data?
@teunschutte does not store your personal data any longer than is necessary to achieve the purposes for which your data is collected and processed. This is described in the consent statement.
All data that does not fall within the statutory retention obligation will be deleted no later than 6 months after completion of the project. The end date of a project is stated in the consent statement.
Sharing personal data with third parties
@teunschutte only provides personal data to third parties if this is necessary to comply with a legal obligation or if clear agreements have been made in advance.
These agreements are recorded in the consent form.
When data is shared, it is grouped and pseudonymised. Pseudonymization is a security measure that makes it more difficult to trace the data back to you.
For research participants (caregiver perspective)

The processing of your personal data is necessary for the performance of an agreement when you have entered into an agreement with us.

When there is a service agreement
In some projects, a service agreement can be concluded to financially compensate for the time invested. If you, as a healthcare provider, have participated in a project that is paid for by a company active in the pharmaceutical or medical devices industry AND you have received compensation for this, it is mandatory to report this to the Healthcare Transparency Register. This notification will be made by the company that commissioned the order.
To ensure that they can comply with this obligation, @teunschutte will share the following data with the company on an annual basis:
Your name
The institution (e.g. hospital)
The nature of the financial relationship (services)
The total amount
Customers and potential customers
